Hard Disk Diagnostic Virus - MBAM-Setup didn't work effectively

I have recently followed the instructions using the MBAM-Setup to remove a hard disk diagnostic virus/malware. It seemed to help, but I have a completely blank desktop and start menu.

I've now followed the OTL and aswMBR instructions. Unfortunately I couldn't upload the txt files, so please help me either to upload or I can email them. Any advice you can offer is much appreciated.

Many of the things to note for us would be:

• Slow computer
  
• Error messages
  
• Fake antivirus alerts or the icon in the system tray
  
• svchost.exe running at 100%
  
• System crashes or blue screen of death
