Reliability Monitor is part of the Windows Reliability and Performance Monitor snap-in for Microsoft Management Console (MMC).
To open Reliability Monitor in Microsoft Management Console
Click Start >> In search box type perfmon >> ENTER.
In the navigation tree >> Reliability and Performance >> Monitoring Tools >> Click Reliability Monitor.
Select the item in the chart below for more details on it.
Note: Look at the chart above to see when and what you had failed. This can be helpful if you don't know what caused the failure but know when, or vice versa.
From here you can see what errors occurred.
