Infection with stranege windows logon screen

I've connected remotely to a friends computer and it had a strange logon screen. When I rebooted it in safe mode it came to the screen to log on to either Administrator or Owner. When I clicked on Administrator it then came to the screen below in the screenshot.
Has anyone experienced this before or how to fix it?

Please download the current version of HijackThis from HERE

• Double click and run the installer.
  
• It will install to C:\Program Files\Trend Micro\HijackThis\hijackthis.exe
  
• After installing, you should get the user agreement, press accept and Hijack This will run.
  
• Select Do a system scan and save a log file. This will open a notepad file of everything Hijack This found, copy and paste it back here.
  

I can't get into a web browser to do anything, I can't by pass that window currently. I can type in that box and what I enter appears.
But can't get past it or get the Task Manager up.
