I/O device error & usb wont format

I have 1 flash drive&1 mp3 flash drive. My flash drive wont open&fails when trying to be formatted&my mp3 wont open and keeps giving messgas like "E:/ is not accessible..could not be performed because of i/o device error"...
ive tried all the intructions on windows troubleshooter but nȯne of them work. Can someone please help me fix these problems.

Please try booting into safe mode to perform the format.

I tried formatting on safe mode but only my mp3 would format....I couldnt fix the I/O device error at all..
everytime i click on the usb...it opens a window and a message comes up saying that it has not been formatted&do you want to format now?...
when I click yes and then format...a message comes up saying that it cannot complete format...
Why wont it let me format and how can I fix this problem?

I think your thumb drive is spoiled or a firmware inside it has corrupted. Do you always remove the device safely?
